What is a Life Care Plan?

A Life Care Plan is a detailed document that outlines the medical, rehabilitative, and supportive care needs of an individual who has suffered a serious injury, chronic illness, or disability. It is typically created by a Life Care Planner, who may be a medical professional such as a nurse, physician, or rehabilitation specialist. A Life Care Plan is important because it ensures the individual receives the necessary care and services for their lifetime, helps families and caregivers plan financially and logistically, and serves as evidence in legal cases for determining future medical and support costs
